We require each player to provide the following documentation prior to taking the field with their respective teams. Team Manager will begin collecting from you in the Summer:
- Adult Code of Conduct
- Athlete Code of Conduct
- PRESENT ORIGINAL BIRTH CERTIFICATE; leave a copy with Team Manager
- REPORT CARD with all four quarters of prior school year - please provide a hard copy
