Welcome to the 2024 Tackle Football Registration with Weston Warriors in Weston, Florida!

This registration session will allow you to complete contact information, register your child, and submit payment. Please have your Credit Card or checking account information available. Payment alone DOES NOT complete the registration process. 

(1) Present Original Birth Certificate, leaving us a copy.

(2) 2X2 Digital Photo (head shot from shoulders up)

(3) Participant Physical signed by Doctor, clearing participation, dated after January 1, 2024 & prior to participation. Form will be available in 2024. If you participate in conditioning, please be sure that your child is medically cleared to do so, as this is your responsibility.

(4) Participant Contract. Form will be available in 2024.

(5) A copy of your athletes 2023/24 School Report Card showing ALL 4 Quarters is also required prior to the beginning of the season. PLEASE keep a copy and provide it to your Team Manager as soon as possible.

(6) Code of Conduct signed.

(7) Equipment Deposit Payment as security for the return of our equipment after the season.

We may have a Doctor available providing physicals just prior to the start of the season for your convenience, but you can use your own Doctor.

***IMPORTANT NOTE: We are always looking for qualified Coaches & other enthusiastic Volunteers to acclimate into various roles in our program. Please fill out the volunteer portion of the registration should you be interested.***

***FUNDRAISING: Our registration fees cover only 2/3 of the cost needed to operate our program. In order to meet the the difference, we require each participant to sell a minimum of $100.00 in raffle tickets. The tickets will be distributed near the beginning of the Regular season. We draw prizes during our Homecoming games ranging from $1,500.00 for 1st place to a Big Screen TV to the participant that sells the most, subject to change each season, with details being shown on the Raffle Tickets when printed. Any shortfall in sales may be deducted from your Equipment / Uniform Deposit payments.

Your registration is not complete until payment is processed, that Transaction Fees may apply, & that when your payment has processed, your Registration Fees are NON-REFUNDABLE
